BEREAVEMENT LEAVE: After completion of 90-day probation, employees will be allowed the following: *Full-time employees are allowed up to 24 hours off with pay per anniversary year. *Part-time employees are allowed 8 hours off with pay per anniversary year. Additional time off without pay may be arranged with your supervisor for funeral attendance, providing the schedule permits.
HOLIDAY PAY:Trego County Lemke Memorial Hospital recognizes the following days as Holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. Employees who must work on any holiday will be paid at the rate of time and one half for those hours.PAID TIME OFF (PTO):Trego County Lemke Memorial Hospital combines vacation and holiday hours into one bank called Paid Time Off (PTO).
PTO Accruals: During the first through fourth year of continuous employment, one hour of PTO is earned for each 15.29 hours worked. (136 hours maximum) During the fifth through tenth year of employment, one hour of PTO is earned for each 11.82 hours worked. (176 hours maximum) During year eleven and after, one hour of PTO is earned for each 10.4 hours worked. (200 hours maximum) PTO is not earned on hours worked greater than 2,080 in a year. Maximum Bank Accruals: Through year four the maximum accrual that can be banked is 272 hours. Through year ten the maximum accrual that can be banked is 352 hours. After year ten the maximum accrual is 400 hours. There will be no pay in lieu of PTO not taken.
You are eligible to use PTO after completion of a 90-day probationary period. Employees may use up to 44% of the PTO hours accrued in first year of employment. Please consult the payroll coordinator for additional information.
SICK LEAVE:Sick Leave benefits are provided to replace wages lost due to time absent from scheduled work because of personal illness, including childbearing, or illness of a dependent. A dependent is defined as a spouse, an unmarried child up to age 18, an elderly parent who lives with the employee, or other situations with administrative approval.
Full/Part Time Employees: Full/Part time employees qualify to collect sick leave after 90 day of continuous employment. Sick leave time, however, begins to accrue from the date of employment. Full/Part time employees accrue sick leave time at the rate of 1 hour for each 17.3. hours worked. Employees can accrue a maximum of 120 hours per calendar year. The maximum allowable bank is 280 hours. Sick Benefit Rules: During your 91st day through your first 1st year, sick leave is payable on your 3rd scheduled work-day of continuous illness. Beginning your 2nd year of employment through your 4th year, sick leave is payable on your 2nd scheduled work-day of continuous illness. Beginning with your 5th year of continuous employment sick leave is payable on 1st scheduled work-day of continuous illness. If more than one illness occurs during a year, the Sick Benefit Rules apply each time.
Conditions for Sick Benefits: Sick benefits are not paid for routine appointments with doctors, dentists or other health professionals, or for travel time to such appointments. Paid Time Off (PTO) and/or time without pay may be used for these appointments, if they cannot be arranged around your work schedule. If sick benefits are authorized for payment, they will be paid according to your years of service and employment status.
LEAVE OF ABSENCE: If you need to request a Leave of Absence, you should discuss this with your supervisor. A Leave of Absence may be granted by mutual agreement between an employee and the Hospital. A Leave of Absence, without pay, may be granted for military service, education, maternity, family or medical reasons or other circumstances when both the employee and Hospital may benefit. For specific information on the continuation of insurance coverage and payment of premiums, contact the Payroll Coordinator.
JURY DUTY: Employees will be paid their base pay for the number of regularly scheduled work hours they miss while serving on a jury. Any jury pay received by the employee must be endorsed to the hospital.
EMPLOYEE DISCOUNTS FOR EMERGENCY ROOM: A 50% discount for emergency room services will be given to the employee.
INSURANCE, DEPENDENT LIFE: A Dependent Life Insurance program is paid by the hospital for all employees who work at least 20 hours per week, and have been employed by the organization for six months. Eligible dependents will be enrolled in Dependent Life Insurance coverage valued at $10,000.00 per eligible dependent. Eligible dependents are your spouse, children ages 6 months to 21 years or 25 years if a full time student. Newborn coverage (age 7 days to 6 months) is $2,000.00.
INSURANCE, GROUP TERM LIFE:A Group Term Life Insurance program is paid by the hospital for all employees who work at least 20 hours per week and have been employed by the organization for six months. The cost of this employee insurance is paid by the Hospital. Eligible employees are enrolled in coverage valued at two times their annual base salary. Benefits decrease after age 65. Separate brochures can be obtained from the Payroll Coordinator.
INSURANCE, HEALTH: Group Health Insurance including dental and pharmacy coverage is offered through Blue Cross and Blue Shield of Kansas to all employees who work an average of 20 hours per week. Single or family memberships are available. Membership is optional, but employees are encouraged to enroll. Enrollment must be done within 30 days of employment or at open enrollment in June to begin on the plan’s anniversary date, July 1. The Hospital shares in the cost of the group health plan. The employee’s share of the premium is paid by payroll deduction. The employee’s portion of the premium will be tax-sheltered under the Section 125 plan. Please contact the payroll coordinator for additional details and costs.
INSURANCE, LONG TERM DISABILITY:A Long Term Disability Insurance program is paid by the Hospital for all employees who work at least 30 hours per week and have been employed by the organization for six months. A separate brochure can obtained from the Payroll Coordinator.
INSURANCE, LONG TERM CARE:A Long Term Care Insurance program is paid by the Hospital for all employees who work at least 30 hours per week and have been employed by the organization for 30 days. A separate brochure can obtained from the Payroll Coordinator. PENSION:To provide for retirement, the Hospital has a mandatory pension plan for employees who work at least 1,000 hours per year, are age 21 years and have been employed by the organization for one year. The plan also allows employees to make additional tax-deferred contribution son a voluntary basis. The contributions to the plan are as follows: Employee 2.5% of first $7,800 of annual salary. Contributions: 5% on annual salary in excess of $7,800. Hospital 4.5% on first $7,800 of annual salary. Contributions: 9% on annual salary in excess of $7,800. A detailed explanation of the Plan may be obtained from the Payroll Coordinator.
ANNUITYTax Deferred Annuity programs are available to full-time and part-time employees through the Hospital payroll system. Employees who are interested in having part of their earnings withheld free of Federal Income Tax and paid into an annuity may contact the Payroll Coordinator.
FLEXIBLE SPENDING ACCOUNTS:All full-time and part-time employees are eligible to participate in Flexible Spending Accounts (FSA’s) established and governed by federal regulations. These FSA’s allow employees to set aside part of their earnings, BEFORE taxes, to pay for work related dependent care and/or un-reimbursed medical expenses that the employee or family members incur. Additional information on Flexible Spending Accounts may be obtained from the Payroll Coordinator.INSURANCE, 24 HOUR RISK ACCIDENT:An accident insurance plan is offered by the Hospital to protect employees against financial hardship caused by accidental death, disability or dismemberment.Employees under age 70, who work at least 16 hours per week, are eligible to enroll in this plan. The employee pay premiums through payroll deduction. The amount of accident insurance can vary depending on the employee’s contract. Additional information may be obtained from the Payroll Coordinator.
INSURANCE, CANCER:Membership in the Hospital’s cancer insurance program is available to full-time and part-time employees who work at least 16 hours per week. Cancer plans provide a wide array of extended benefits. You must sign up within 30 days of eligibility. Additional information can be obtained from the Payroll Coordinator.
INSURANCE, ADDITIONAL ACCIDENT CANCER & HEART PLANS:Many additional insurance benefits are offered to all employees including additional accident, cancer, heart-stoke, and intensive care and premiums are paid through payroll deduction. Additional information can be obtained from the Payroll Coordinator.
INSURANCE, UNIVERSAL LIFE:Universal Life Insurance is offered thru payroll deduction to all employees during the open enrollment period held once a year. The amount of life insurance coverage can vary depending on the employee contract. Additional information on Universal Insurance can be obtained from the Payroll Coordinator.
