From 13th Street, please turn east onto Warren Avenue.  Proceed one block to 14th Street, turning North for 1/4 of a block.

are located behind the hospital.

From 13th Street, please turn east onto Easter Avenue.  Proceed 3/4 of a  block to the entrance.

Ellis Family Care

is located at 1000 Washington Street. From I-70 take Ellis Exit, Highway 247.  Turn south onto Washington Street.  The Ellis Family Care Center is located  at the corner of Old Hwy 40 and Washington Street.

are located in the same facility.  We are located on the east side of town known as  320 N. 13th Street or Hi-Way 283 North just north of the high school.  Please enter by the entrance off of 13th Street.
